On the main landing page for this campaign I think a simple poll is best:
I've had a failure which was covered under warranty
I had a failure that was not covered under warranty
So far I'm problem free

However also I'll set up a few more polls to collect more data, like...

How about something along the lines of how helpful the dealer and/or Nissan was with replacing the rad, transmission and the length of time it took? Difficulties encountered? This would be helpful to know both pre and post rad warranty extension.

It would also be helpful to know when, as in what situation, the driver and vehicle were in when they discovered the failure. Was it on the road, at a shop, etc?
