VIAS Power Valve failures

Hey folks. How many people have had issues with the "power valves" in the intake manifold runners (VQ35) coming loose and being ingested by the engine? If so, what kind of mileage? My 04 R50 just spit them out and destroyed #6 cylinder at 99,800mi. Did a valve leakage test and it fails miserably. 60% leakage. I'll be fully dis-assembling the engine tonight. Im expecting to find one of the screws hammered into the cylinder head or valve seat as it has a very loud tap in the top end after retrieving the 3 valves that were lodged in #6 and replacing the missing screws with the updated ones with the small washers. wish me luck. should be able to post some pics later tonight

Most of the ones I've heard about occurred on 01-02 models. It's a shame to see so much damage caused by a lack of a little threadlock compound!
